Potholes/surface break up - REPEATED, FAILED REPAIR

Sent to Buckinghamshire Council 3 minutes later. FixMyStreet ref: 2017203.
This pothole was "repaired" only about two weeks ago... but all sign of the repair has gone - actually the "repair" started failing on the day it was done.
The failed surface is now worse than it was before it was 'repaired' two weeks ago.
I would like confirmation that no public money has been incurred because of this failed repair - that the contractor (if it was an external contractor) will be made to redo the repair at their own expense. If it was done inhouse please confirm that the cost of the repair will be deducted from the relevant directors salary.
No public money should be incurred in redoing this 'repair' and doing it properly. Whoever was responsible for the 'repair' should pay.
