Pothole/surface fail around ironwork of drain

This drain's surroundings have broken away to reval a pothole around the ironwork. This was reported and repaired approx 2 years ago. It is now back again and dangerous for cyclists and cars. I believe cyclists have been injured/killed before due to road surface issues exactly like this, in the UK in previous years. This stretch of road now shows failure in multiple locations (particularly surrounding drains/manholes)
