Collapsing road and large pothole in dangerous location

Affinity Water surveyed the road in late December and found a defect under the road outside Number 45 - they said they would return within 20 days to dig up and fix this (which has not happened).
The area within their blue spray painted square has now started to sag and a large pothole has formed alongside it - which is getting larger as the stones within it are scattered by cars. Cars driving through are very noisy and when it snows/is icy the pothole fills with black ice and is very hazardous. As it is on a bend it cannot be avoided by road users and is impossible to avoid it when driving in/out of the driveway.
