Metal plate protruding from sand

For over 3 years there has been missing paving stones outside my house.. two were replaced some months (18-24 ) but 1 was replaced with some sort of sand composition... this has now eroded and a metal plate it sticking out which will if not already be a trip hazard...

Thank you for your report. Although this does not meet our criteria for requiring immediate attention, this defect will be added to our records for the future.
Our records will be used to generate larger planned works and will also enable us to pick up this defect when we are carrying out repairs of a similar nature in the area.
